A theoretical math question?

My boyfriend and I are having a discussion about staffing levels in a hospital ER. As most ERs are understaffed, my theory is that for every 1 patient there should be 3 doctors available among say 200,000 patients seen on a daily basis. However, knowing the staffing situation, the ratio seems to be 2 to 1. But based on certain factors, we know that some things only work in theory than in reality. So is my theory close?
